This form is sent by Hackney LAC Education team to each school that has Hackney LAC on their roll usually in July/September every year for a return in October.

It is completed by the designated teacher and the confidential information contained in it is made anonymous and summarised to provide data for Government returns on the educational achievements and related areas eg attendance for children in Hackney's care.
